Repatriation of Remains and Ashes

Diplomatic and consular representations assist with the repatriation of the remains or ashes of deceased Italian citizens abroad. The procedure for repatriation involves requesting authorisation from the relevant Italian Municipality for burial and the issuance of a mortuary passport.

Documents for Repatriation of Remains to Italy:

  • Death certificate issued by the Philippine Statistics Authority (PSA), apostilled by the Department of Foreign Affairs (DFA), and translated into Italian;
  • Medical certificate stating the cause of death and, if applicable, an autopsy report;
  • Original letter from the local health authority;
  • Original and a copy of the certificate of authorisation for the transport of the remains to Italy issued by the competent local authority;
  • The deceased person’s last passport;
  • A letter of authorisation signed by the family specifying the cemetery and Municipality where the burial will take place. The consular office will request the authorisation for the repatriation of the remains from the Municipality. The letter should also authorise the funeral agency to act on behalf of the family in dealings with the consular office.
  • The remains must be properly treated (embalmed for transport) and placed in a coffin suitable for international air transport (internally sealed with zinc, with sand in the bottom, etc.), with the following details: the deceased’s name, surname, date of birth, and date of death;
  • Embalming certificate;
  • Indication of the airport of entry for the remains into Italy, as well as full details of the flight;

The consular office will affix seals to the wooden coffin.

A consular fee is required for the issuance of the mortuary passport, which must be accompanied by the full documentation listed above.

Documents for Repatriation of Ashes to Italy:

  • Death certificate issued by the PSA, apostilled and translated into Italian;
  • Medical certificate stating the cause of death and, if applicable, an autopsy report;
  • Two original certificates of cremation issued by the crematorium;
  • Original letter from the local health authority;
  • Original and a copy of the certificate of authorisation for the transport of the ashes to Italy issued by the competent local authority;
  • The deceased person’s last passport;
  • A letter of authorisation signed by the family specifying the cemetery and Municipality where the burial will take place. The consular office will request the authorisation for the repatriation of the ashes from the Municipality. The letter should also authorise the funeral agency to act on behalf of the family in dealings with the consular office;
  • The cremated remains must be placed in a suitable urn, with the following details: the deceased’s name, surname, date of birth, and date of death, and placed in a wooden container for transport;
  • A metal plaque bearing the deceased’s name, surname, date of birth, and date of death must be affixed to the wooden container;
  • The plaque should be glued securely to the container, and the wooden case must be fastened with metal bands, one lengthwise and one widthwise, with the bands at least 2 cm wide.

The consular office will affix seals to the wooden box.

A consular fee is required for the issuance of the mortuary passport, which must be accompanied by the full documentation listed above.
